IMPACT: This mechanism is designed to assist with the scale-up of high quality and innovative HIV treatment services at Lighthouse, and through demonstrating new service delivery and integration models, is also designed to improve the quality and efficiency of HIV treatment services nationally. The Lighthouse is the largest single provider of integrated HIV services in Malawi, supporting over 25,000 PLHIV. It provides tertiary level referral services at two clinic sites: the first clinic, opened at Kamuzu Central Hospital in 2002, and subsequently the Martin Preuss Centre (MPC) established in 2006 at Bwaila Hospital. MPC integrates ART with TB service delivery at the largest TB treatment site in Malawi and supports the provision of Option B+ services at the Bwaila Maternity Unit, the highest volume maternity unit in Malawi. It has been designated a National Training Center by the MOH and certifies providers in HIV testing and counseling (HTC), palliative care and ART provision. Lighthouse program models inform national policy and provide operational guidance to improve the quality and efficiency of integrated treatment, care and support for HIV service delivery and facility and community level. The partner collaboratively pioneered the development of an Electronic Data System (EDS), which has since been rolled out to over 40 of the higher burden ART sites nationally, with further roll-out planned. It has also launched an innovative program aimed at reducing defaulters and improving long-term retention in ART treatment through active tracing and follow-up. Lighthouse was also one of the three sites chosen to pilot routine viral load screening, and is currently using its lessons learned to help coordinate the roll-out to over 70 sites nationally.
