In COP 14, State will continue to advance its focus on the transition of direct services to the host country and support sustainability and country ownership through targeted technical assistance and capacity building. State will support the country team in moving forward the recently revised Transition Plan and will continue to engage the government, civil society, private sector, donors and other stakeholders in the HIV response.

Strategic information will continue to be a priority for State. State will coordinate and support the implementation of the new PEPFAR Monitoring Evaluation and Reporting Strategy, aligning its M&E framework with the new guidance and support the partners in understanding and incorporating the new guidance into their existing systems. State will also support the development of a PEPFAR facility list and engage the relevant stakeholders in developing a Master Facility List.

To accompany the revised Transition Plan, State will work with the interagency team to develop a monitoring plan for the Transition. The framework will focus on monitoring various elements of the transition:

• Continued monitoring and reporting on indicators for PEPFAR supported activities

• Monitoring of specific transition processes

• Monitoring of country ownership

• Monitoring the maintenance of quality service delivery during and after transition

To ensure that essential data remains available to PEPFAR as we transition from direct services, State along with the interagency team will continue to build its partnership with the Ministry of Health and other stakeholders to support and work towards one National M&E system. State will also continue to collaborate with the Ministry of Health on all relevant national activities e.g. strategic plans, World AIDS Day etc.

Since COP2014, PEPFAR no longer produces narratives for every mechanism it funds. However, PEPFAR has now included performance targets or indicator information for each mechanism based on the Monitoring, Evaluation, and Reporting (MER) system. The MER guidance is available on PEPFAR's website https://www.pepfar.gov/reports/guidance/. Note that COP years 2014-2015 were under a previous version of the MER system and the indicators and definitions may have changed as of the new 2.0 guidance.
